A Retired General City Employee Under Age 65                      

            Who retired before 1/1/2005

            Who retired after 1/1/2005

A Surviving Spouse                      

An Early Retirement                       

An Ordinary Disability Retiree                 

A Duty Disability Retiree, and                 

            A Fire Fighter
