Address Changes

To change your address:

You must notify the Employes' Retirement System of your address change in writing.

You can use the attached Address Change Form or send notification including:

• both your new and old addresses  
• the date your address change becomes effective  
• your Social Security Number  
• your signature  
• the date the notification was signed.
