Pension Beneficiary Changes

Births, deaths and divorce are some of the reasons why you may want to change your designated pension beneficiary. 

Retired members - You cannot change your named survivor, but may change your contingent beneficiary using this form.  Please call and speak to a pension specialist if further guidance is needed.

Active or Deferred Members - Review the current beneficiary information listed on your account statements.  To change a beneficiary, you must complete a Beneficiary Change Form, have it witnessed by two persons not listed on the form as beneficiaries, and return the form to the Employes' Retirement System. This will affect your pension benefits only. A separate beneficiary form is necessary for life insurance benefits.

You may get a copy of the Beneficiary Change Form here, have a Beneficiary Change Form mailed to you, or you may pick one up at the Employes' Retirement System office, located at 789 North Water Street, Suite 300.  You can contact the Employes' Retirement System at 414-286-3557 (or toll free at 1-800-815-8418).
